Request Incident Report

The following page is a step by step way to request an official fire report

Send a formal letter to  : Wright Township Volunteer Firemens Association

The letter MAY ALSO BE EMAILED TO THE DEPARTMENT AT   -   info@wtvfd.com (see point 5 below) - Subject Line: Incident Report Request

The more information provided, the easier and normally quicker it will processed. PLEASE, ensure all information is accurate. (1)

If you are an Insurance Company, Legal Counsel or Business:(6)

Please include - Name of Entity requesting the report, Reason for requesting the report, Date of the incident, Type of incident (Fire, Vehicle Accident, Etc), First Name of person involved in incident if available, Location of Incident, YOUR contact information (Entity Name, Mailing Address, Phone Number, Fax Number and Email Address) and any other information that may help us locate the report correctly.

How you would prefer the Report be sent and at least one alternate delivery method (2)

A date when the report is needed by (1)

A return, prepaid envelope able to accept a minimum of 3 pieces of paper (per each incident).  - This is not necessary if returning by email or fax(3)

A processing fee of $25 made payable to (DO NOT SEND CASH) : Wright Township Volunteer Firemens Association or Wright Township Volunteer Fire Department  (4)

If you are emailing your request - please remit the processing fee with the same information (Print the email you sent) (5)

 

If you are calling on behalf of your insurance company, legal counsel or Business:(6)

Additional Information

You may also call the department at 570-868-3765 and follow the voicemail prompts for REPRESENTATIVE , this will not speed up your request.

(1) We are a 100% Volunteer Organization, the processing of information is sometimes slower because of this, time may vary in processing requests.  

(2) All information provided must be valid and accurate, if it is not deliverable, we will NOT repeat attempts or waste time trying to get a request processed. 

(3) This may vary from 3 pieces of average letter sized paper depending on the scope and size of incident and number of incidents requested, disregard if returning by email or fax

(4) This fee is the basic fee, it may vary depending on the amount of information requested.

(5) If you are sending an email, you do not have to include all of the information in the email body, however, all the information must be included in a formal written request letter.  When sending the processing fee,simply print the letter and include it with the processing fee. Include a subject line of Incident Report Request and the date of the incident.

(6) This Association may reject an offer for a written report at any time.
